FEES AND PAYMENTS
We attempt to provide healthcare in a cost-effective way. Our fees are comparable to other specialists in the area. Please contact our billing office to discuss any questions you may have regarding fees or services.
FOR OUR PREGNANT PATIENTS:
For your convenience, we will contact your insurance company and review your benefits. We will give you a copy of your benefit details along with your monthly payment information, if required.
If you are currently covered by OHP call 1-800-699-9075 to notify them that you are pregnant.
If you are going to apply for the Oregon Health Plan (OHP) please submit your application as soon as possible. Central Oregon Perinatal Care can assist you with your application for OHP, as well as resources such as WIC, counseling, transportation and more. You can reach them at 541-322-7448 or www.deschutes.org/prenatalscreen, to fill out their intake form and someone from Central Oregon Perinatal Care will contact you.
To apply for Paid Leave Oregon, use the link above and click on the green "Apply Now" button at the top of their website.

You can click on the button at the top to enter your email and start the process. Once your request is complete, you will receive an email with a secure link to download your records.
How long does it take to process requests?
Records can be available for download in as few as 8 business hours! However, some records may be subject to physician approval and can take up to 5 business days. If it has been more than 5 days since you made your request, please reach out to HealthMark at status@healthmark-group.com for an update.

You can email to records@ecwg.net, fax to 541-389-8115, or drop off at our clinic.
Requests may be submitted electronically to HealthMark’s Request Manager through the link above. Once logged in, select “Submit Request” from the menu options and enter all required fields to submit a valid authorization. Each form requested for completion will require a $35 fee to be paid directly to HealthMark. Estimated turnaround time will be 7 business days.
If an email is provided, you will receive a response directly from HealthMark notifying you of the prepayment invoice. After payment, the completed forms may be downloaded directly through HealthMark’s website, or provided directly to you through an agreed to delivery method (i.e. secure email, mail, etc.).
